79 year old male with symptoms of Stage IV CHF: dyspnea, ascites, basilar crackles and fatigue.

Past Medical History: History of CABG, angioplasty, biventricular pacing, AAA repair and frequent hospitalizations for CHF.

Admission Labs: Creatinine 2.3 mg/dl, Hct 38.6, K 3.3

Medications: Nesiritide, Dobutamine, Lotensin, Demadex, Renexa

Treatment:

  • Venous access obtained via a mid-line catheter and angiocatheter
  • Diuretics withheld during Aquapheresis treatment
  • Fluid removed at a rate of 180 mL/hour
  • Total of 3.6 liters were removed via Aquapheresis in 20 hours
  • The patient urinated 1.6 liters during the procedure
  • The procedure was well tolerated.

Results:

  • Patient admitted, treated, and discharged within 24 hours.
  • Upon discharge, the creatinine level had decreased 0.5 mg/dl to 1.8 mg/dl and oral diuretics were withheld for the next 48 hours.
  • Subsequently, oral diuretic doses were 50% of the pre-Aquapheresis dose for the next 48 hours and then decreased an additional 50%.
  • Seven days post treatment, patient on 25% of the pre-treatment diuretic dose and has had no weight gain.
